Commonwealth Home Support Programme

The Commonwealth Home Support Programme (CHSP) funded by the Australian Government Department of Health helps eligible older people living in the community to maximise their independence, through the delivery of timely, high quality entry-level support services.

Services promote wellness, enablement and restorative care.

Eligibility

These services are available to any resident in the Eastern Metropolitan Region who is:

• 65 years or older,
• 50 years or older and identify as an Aboriginal and/or Torres Strait Islander person, or
• 50 years or older and on a low income, homeless or at risk of homelessness.

Services

MannaCare offers a range of respite, allied health and therapy services under a funding agreement with the Department of Health to provide good quality respite care, allied health services and supports caring relationships.

Set in the beautiful grounds of MannaCare, Grevillea House is a purpose-built, cottage-style building that provides many facilities and resources to support on-site respite needs.

From social support to overnight cottage respite, MannaCare is able to deliver excellent care through experienced staff, with compassion and respect.

MannaCare social support day programmes are available for frail older people and those living with dementia.

These programmes are funded by the Commonwealth Home Support Programme (CHSP). MannaCare charges fees in line with CHSP guidelines.

Social Support programmes have a dual purpose:

  • to provide social support for people
  • to assist older people to participate in the community

They also enhance clients’ quality of life, while maintaining their social interaction and skills in a supportive environment.

Programmes that operate from Grevillea House include a range of social activities that are designed to meet clients’ individual goals and care needs. An ‘Active Service Model’ & ‘Montessori’ approach is embedded in all programmes. This approach aims to support a person’s strengths and promote independence.

Assistance with transport is available by assessment, and a fee applies.

Our Grevillea House cottage respite is for people over the age of 65 or 50 if ATSI, who have been assessed as eligible to access the overnight cottage respite program.  The service is deliberately flexible and responsive, which allows clients and carers to plan their respite stay how it best suits them.

For instance, respite might be needed for a single night, or a week, or two (60 days per year maximum).  Accommodation is in a purpose-designed and built extension to Grevillea House which was opened in 2007, and is styled like a motel.

We have five single rooms, each with an ensuite, available to residents of the Eastern Metropolitan Region. The service is available 7 days per week, excluding a two week closure at Christmas.

This is available for consumers to help them balance care responsibilities. We provide this support in the client’s own home only. Services are offered 7 days per week for those living in the Eastern Metropolitan Region, and are structured to respond to the consumer’s needs.

Cassia Day Respite

MannaCare offers a range of services including a residential respite program, Cassia House Day Guest Respite Service.

Centre-based respite services

Cassia House hosts a day respite service for frail older people. It enables carers to have ‘time out’ in a flexible manner that is responsive to their needs.

The service operates on Thursday, Friday and Saturday, and it can cater for up to 10 clients at a time. Clients are able book in for either a half or full-day session, with all meals provided. A daily fee applies.

The programme closes for 2 weeks over the Christmas/New Year.
