Commonwealth Home Support Programme

The Commonwealth Home Support Programme (CHSP) is an initiative funded by the Australian Government Department of Health that aims to assist older individuals in maintaining their independence while living in the community. The program offers a range of high-quality entry-level support services designed to promote wellness, enablement, and restorative care for eligible older people.

Eligibility

These services are available to any resident in the Eastern Metropolitan Region who is:

• 65 years or older,
• 50 years or older and identify as an Aboriginal and/or Torres Strait Islander person, or
• 50 years or older and on a low income, homeless or at risk of homelessness.

Our Program

The Centre-based Respite and Social Support Services provide a range of tailored scheduling options to meet individuals’ specific needs.  Services are operational from 10:00 a.m. to 3:00 p.m., Monday through Friday, and offer flexible arrival and departure times.  These services are also designed to support frail older individuals and those living with dementia, considering their unique requirements.

Our programs feature a variety of holistic activities and recreational options specifically tailored to address each client’s unique goals and care needs.  We integrate an ‘Active Service Model’ and ‘Montessori’ approach into all of our programs, focusing on leveraging individuals’ strengths and fostering their independence.
